School Nurse Survey
2015-2016 School Nurse Survey
Act 935 of 2015 requires all districts to complete and submit the School Nurse Survey. This survey is in two parts. This data will also provide you the information that Act 935 requires you to report to your school board.
The School Nurse Survey for 2015-2016 is now available. This year, as in years past, Part 1 of the survey is about the School Nurse. Part 2 is about the students.
The data you have reported for the past two years has been presented to the joint Legislative Public Health committees, joint Legislative Education Committees, Public School Health Services Advisory Committee, Child Health Advisory Committee and the Task Force addressing Special Education best practices. In addition, the data will be presented on July 21, 2016 during Grand Rounds with the AR Department of Health. I want you to know that your data is important and is being heard. So please be very diligent in your data reporting. Be accurate, don’t guesstimate anything.
When you get to the end of the survey, BEFORE you click submit, it will provide you a summary of what you have entered with the option to print it as well. DO NOT SUBMIT unless you have provided all of the information. If you only have part of the data, you can fill in the data BUT you will have to look at the top of the page. There is a gray stripe across the top of each page. It says “SAVE BUT DO NOT SUBMIT”. This will save the data you have entered and send you an email with a link in it. This link is to your saved incomplete survey. Click on the link in that email and finish entering the data. If you do not get that email, check your spam or junk mail folder. But PLEASE do not submit until you have entered all of the data. I delete all incomplete surveys.
I also track and have to report which districts completed the survey so please make sure to enter your data.
