Here it is! The 2012-2013 School Nurse Survey!


IMPORTANT INFORMATION!     PLEASE READ CAREFULLY!

 

Nurses,

I have taken your comments, suggestions and complaints about the survey and tried to improve it. I have tried to make it more user friendly and less cumbersome while still trying to collect data that can be reported to others. This year the survey will be in 2 parts. Remember, you have the ability to save the data you have entered, but not submit it until you are through entering data. THEN, for part 2, DON'T submit until the end of the school year. We have 6 weeks left of school. 

Part 1 of the survey includes the demographic data about you and your school CAMPUS. It will take 8-10 minutes for you to complete. Once you have entered ALL OF THE DATA, then you can submit Part 1. It is extremely important that you complete Part 1 for EACH campus you serve, DO NOT add them together.

Part 2 will entail all of the student data that is gathered each year.  You will need to complete Part 1 and Part 2. 

I will never be able to say THANK YOU enough to Leon Brown, Technology Coordinator for Crowley's Ridge Educational Cooperative. Leon Rocks!  I also hope to have a student from the College of Public Health to help with some statistical analyses with the survey data.

Remember, documenting what you do will be VITAL in recognizing all that you do on a daily, weekly and monthly basis. In addition, share your data with your administrators and school board. When I presented to the Arkansas School Board Association, I encouraged them to go back and ask you to report to the board during the summer. Give them an accurate picture of the health of your district.
