Here it is! The 2011-2012 School Nurse Survey!

IMPORTANT INFORMATION!     PLEASE READ CAREFULLY!

 

• You may enter data anytime, just REMEMBER to save but do NOT submit until the end of the year.  You will be asked to enter your email address and then when you save your data and do not submit, you will receive an email message with a link in the message. Follow the directions. When you click on the link, it will return you to your saved survey data. Just remember to save but don't submit each time.
  

• I have made the survey one continuous page. After you click on cooperative, then district, then school campus, the rest of the survey is one continuous page. Enter your cooperative, district and campus, then look at the rest of the survey so you will have a good idea about what has been added.




• When it asks for numbers please enter numbers only! Text and commas cannot be counted.

• There are some new items related to AEDs, School flu clinics and immunizations, DNRs, emergency (rescue) medications.
